In terms of the ground cover why not a hybrid approach? I'm assuming you mean foliage? If so maybe determine the method based of distance use instanced geometry closer to the camera and paging the farther away from the camera. With large chunks of distant grass they could tend to be on screen longer than those closer to camera while anything closer can be on and off screen rapidly.
