Morrowind allows making marks on the map with a description of up to thousand characters or so. Just double click the map and you will get a dialogue with a multiline text input field.

As far as i know such icons are rarely scaled, for better readability authors usually supply versions for every needed size which requires a lot of manual adjustments even if the sources are in vector format. While you may decide to scale anyway because it is so much more work to draw all those icon...
