Well people see it as one in some cases. In the original game it isnt. People using permanent waterbreathing effects can still accomplish the quest by taking some damage health poisons or other methods to hurt themselves. As long as their Z position is below the requisite height, and that their health is low, it'll work. Inversely, you can drown for hours to no avail if you have a constant regeneration effect that heals you faster than drowning damages you.
