Probably nothing you can do on your own, this logic is handled by C++. This detail hasn't been given as much thought as it should have because it was an unilateral addition at the time. OpenMW is an engine first and foremost and doesn't include any playable content. That footage is from OpenMW Example Suite. Try that out.

Hrnchamd suspects that the software audio rendering Morrowind has is based on salvaged Daggerfall code, but there's nothing concrete to back it up with.

The index depends on your content language (encoding). In the launcher, pick the language that corresponds to your content files (mind that anything that isn't Polish or Russian is Win-1252, Polish is Win-1250, Russian is Win-1251). This has been tracked under issue #4811 for over 3 years. Not something trivial to approach, OSG doesn't have anything similar.

Note that this used not to be the case because 2D voiceover wouldn't have had loudness data analyzed before as late as .46.
